Get gift cards for staying up to date with your health care. After your health care visit, fill out a voucher and a gift card will be mailed to you in 4-6 weeks. You can choose a Target or Walmart gift card.

Print and mail vouchers 

Printed vouchers are also available. Complete your visit or service then print the voucher. Return the completed voucher to Hennepin Health by mail or bring it to the Member Service Center. We will mail your gift card to your home within 4-6 weeks. The section, “to be completed by provider,” no longer needs to be filled out by the provider. Please include the provider’s name instead of the signature.

Child & Teen Checkup visits

Members can earn a gift card for your child's checkup visits:

• $75 gift card for six visits before 15 months at 0-1 month, 2 months, 4 months, 6 months, 9 months and 12 months

• $25 gift card for each visit for each visit at 15 months, 18 months, 24 months and 30 months, and each year ages 3-17

Adolescent immunization

Members can earn a $25 gift card for completing one meningococcal vaccine (MCV), one Tdap vaccine and two HPV vaccines before age 13.
