For the past 2 years, Hennepin Health has worked with Hennepin Healthcare to reduce hospital readmissions for members experiencing homelessness. An analysis showed that housing instability was a major factor that contributed to readmissions within 30 days.
With this program, member at risk of hospital readmissions were identified and connected to social services like housing, transportation and food assistance. As a result, hospital readmissions went down by 3.7% in a 1-year period.
America’s Essential Hospitals awarded Hennepin Healthcare the 2021 Gage Award for Quality Improvement for this program.
