Housing Stabilization Services

PROVIDERS AFFECTED 
Providers participating in the new housing stabilization benefit in designated state programs.

KEY POINTS

  • Hennepin Health will not contract directly with providers for the service, but will work with any MHCP approved provider.
  • Housing stabilization providers should get set up in the Hennepin Health claims system to coordinate reimbursement for services provided.

BACKGROUND
Housing Stabilization Services is a new state Medicaid benefit for people with disabilities and seniors launching July 2020. Housing Stabilization is a Home and Community Based Service (HCBS); providers must abide by the HCBS requirements. All housing stabilization providers must enroll with MHCP and be approved for housing consultation and/or housing transition and sustaining services from the Minnesota Department of Human Services (DHS).

At this time, Hennepin Health will not directly contract for Housing Stabilization Services.  Hennepin Health will accept approved services from any MHCP-enrolled housing stabilization provider. All Housing Stabilization Services will be authorized by DHS; Hennepin Health will be notified through MN-ITS mailbox.

To receive reimbursement of authorized Housing Stabilization Services for Hennepin Health members, all housing stabilization providers should verify that they are appropriately entered into our claims system. Please contact Kue Yang Thao at kue.yangthao@hennepin.us or via fax at 612-466-9344.
