Facility/organization credentialing

Providers and their practitioners must be credentialed before they are considered part of our network. Credentialing is done in tandem with the contracting process.

These providers require facility/organization credentialing:

  • Ambulatory surgery centers 
  • Community mental health centers 
  • County-contracted behavioral and mental health 
  • Home health agencies
  • Hospitals 
  • Nursing facilities/long-term care facilities 
  • Public health organizations 
  • Psychiatric regional treatment facilities 
  • Residential mental health settings  
  • Rule 29 certified mental health centers and clinics 
  • Child behavioral health (inpatient and outpatient within a facility setting)
  • Intensive residential treatment services 

Start the process

Allow 30-90 days for processing (a longer time is needed if a site visit is required).

  1. Fill out the Facility credentialing application. 
  2. Submit the form via email to hhcredentialing@hennepin.us.

You must submit:

  • The complete form.
  • A copy of all current state licenses or an explanation that state licenses are not required.
  • The onsite governmental licensing agency survey with a corrective action plan if deficiencies were cited, and/or cover letter or email from licensing agency stating the facility is in substantial compliance with licensing standards.
  • Current commercial general liability AND current malpractice insurance with Hennepin Health listed as certificate holder.
  • A current accreditation letter or certificate, as applicable.

To check the status of your credentialing application, email hhcredentialing@hennepin.us. Please include your tax ID and NPI(s).

Site visits

We conduct quality assessment site visits for provider location(s) that have not had an MDH or DHS site visit within the last 36 months, or who are not accredited through approved agencies as listed in the Provider Manual. We also conduct site visits for new locations opened under an existing contract where the provider policies have changed or new service were added.

In the event that a provider has satellite facilities which offer the same services and are held to the same policies, a site visit can be performed solely on the primary location.

Update a location

  • Option 1: Complete the Location add/term/change form and email to hhcredentialing@hennepin.us.
  • Option 2: Use the Location and practitioner template roster if making multiple changes. Submit the completed form to hhnetworkmanagement@hennepin.us.

A member of the Provider Operations team will contact you to confirm the update.

Recredentialing

We require facility/organization recredentialing every 36 months. The recredential process AND Credentialing Committee approval must be completed by the last day of your due month in order to remain in good standing.

We notify facilities/organizations of recredentialing 180 days before their due date.

What to do

  1. Fill out the Facility credentialing application (this form is also used for initial credentialing)
  2. Attach the supporting documents listed within the application.
  3. Email the completed form to hhcredentialing@hennepin.us.
