Hennepin Health takes an integrative approach to health care by addressing not only its members' medical needs, but also their behavioral health and social service needs. Eligible Hennepin County residents can enroll in one of three products: Hennepin Health-PMAP, Hennepin Health-MNCare or Hennepin Health-SNBC.

Hennepin Health-PMAP

Hennepin Health-PMAP (formerly called Hennepin Health) is a managed care program that offers medical, behavioral health and social services for those who are:

  • A resident of Hennepin County
  • Between the ages of 0 and 64
  • Eligible for Medical Assistance (Medicaid)

Hennepin Health-PMAP is provided in partnership with NorthPoint Health and Wellness Center, the Hennepin County Human Services and Public Health Department, and Hennepin County Medical Center.

Hennepin Health-MNCare

MinnesotaCare is a managed care program that covers health care for people who do not qualify for Medical Assistance (MA) or receive affordable employer-sponsored health care coverage. MinnesotaCare is similar to MA, except that it requires a monthly premium.

Hennepin Health-MNCare (formerly called Hennepin Health) offers medical, behavioral health and social services for those who are:

  • A resident of Hennepin County
  • Between the ages of 0 and 64
  • Eligible for MinnesotaCare

Hennepin Health-MNCare is provided in partnership with NorthPoint Health and Wellness Center, the Hennepin County Human Services and Public Health Department, and Hennepin County Medical Center.

Hennepin Health-SNBC

Hennepin Health-SNBC (formerly called Cornerstone Solutions) is a Special Needs Basic Care (SNBC) plan for those living with disabilities who are:

  • A resident of Hennepin County
  • Between the ages of 18 and 64
  • Eligible for Medical Assistance (Medicaid)
  • Certified disabled (by a State Medical Review Team or through Social Security Disability Insurance)
