Hennepin Health-PMAP

Hennepin Health PMAP is a managed care program that ensures access to the care and services our members need. If you live in Hennepin County and are eligible for Medical Assistance (MA), Hennepin Health allows you to choose from a wide variety of services and providers within the Hennepin Health network.

Who can enroll

PMAP is for adults without children, pregnant women and families with children.

You must:

Be a resident of Hennepin County
Be under age 65
Meet the income and asset guidelines outlined by the Minnesota Department of Human Services (DHS)

How to apply for coverage

You can apply for coverage online. Complete the application for Minnesota Health Care Programs at mnsure.org. You may also call or visit the Hennepin County Human Service Centers.

What to do at mnsure.org:

Create an account.
Sign into your account. You can then choose to apply with financial assistance.
Fill out the application, review it and sign it.
Submit your application. A screen will appear that tells you if you are eligible for assistance.

If you qualify, you will be contacted by Hennepin County, DHS or your tribal agency to choose a health plan. You may be able to request Hennepin Health for your health coverage.

Cost and benefits

No monthly fee

You do not have to pay a monthly fee (premium) for this plan. You do not have to pay a small dollar amount (co-pay) when you see the doctor or get a prescription filled.

Health-care coverage includes:

Medical care – doctor visits, hospital services and stays, emergency care
Dental care – regular and deep cleanings, check-ups and fillings
Preventative care – vaccinations, screenings and flu shots
Prescription drugs
Home care and nurse visits
Eye exams and glasses
Treatment for mental/behavioral health, and alcohol and chemical dependency
