Plan products

We offer health-care coverage to Hennepin County residents who are enrolled in a Minnesota Health Care Program (Medical Assistance and MNCare). Medical Assistance is a state insurance program that helps people with lower incomes or disabilities.

Our health plans cover adults, children and families. Each has different income or age requirements.

For adults – we offer Hennepin Health-PMAP and Hennepin Health-MinnesotaCare.
For families and children – we offer Hennepin Health-PMAP and Hennepin Health-MinnesotaCare.
For people with disabilities – we offer Hennepin Health-SNBC.

Preventative, routine and follow-up care protects and improves your health. Our rewards program gives you added incentive to get regular care for yourself and your family.

• Earn gift cards for staying up-to-date on checkups, screenings and shots.
• Pregnant women who complete prenatal and postnatal visits earn rewards.
• Eligible children can get free car seats.

Health services

We understand that life can sometimes get in the way of staying healthy. Getting there and knowing more can help you take charge.

Our members can:

• Get rides to appointments and pharmacy pick-ups.
• Talk to a nurse 24/7 through HealthConnection.
• Use the Healthwise® Knowledgebase online resource to check symptoms or learn about your health.
• Come to the Hennepin Health walk-in center to talk in person about what you need.

We take an integrated, community-based approach to health care. We collaborate to offer a health-care model that coordinates with the services and programs offered in the Hennepin County system.
