Hennepin Health-MinnesotaCare

Hennepin Health MinnesotaCare (MNCare) is a managed care program that brings a full network of healthcare services within reach at an affordable monthly premium that is paid to the state. If you live in Hennepin County and are eligible for the State MinnesotaCare Program– we have you covered.

Who can enroll

MinnesotaCare is for adults and families with children who don't qualify for Medical Assistance.

You must:

Be a resident of Hennepin County
Meet the income and asset guidelines outlined by the Minnesota Department of Human Services (DHS)

How to apply for coverage

You can apply for coverage online. Complete the application for Minnesota Health Care Programs at mnsure.org.

What to do at mnsure.org:

Create an account.
Sign into your account. You can then choose to apply with financial assistance.
Fill out the application, review it and sign it.
Submit your application. A screen will appear that tells you if you are eligible for assistance.

If you qualify, you will be contacted by Hennepin County, DHS or your tribal agency to choose a health plan. You may be able to request Hennepin Health for your health coverage.

Cost and benefits

Monthly fee

Your monthly fee (premium) depends on your income and family size. You may have to pay a dollar amount (co-pay) for some services or when you get a prescription filled.

Health-care coverage includes:

Medical care – doctor visits, hospital services and stays, emergency care
Dental care – regular and deep cleanings, check-ups and fillings
Preventative care – vaccinations, screenings and flu shots
Prescription drugs
Eye exams and glasses
Treatment for mental/behavioral health, and alcohol and chemical dependency

Rides to and from medical appointments and pharmacy pick-ups
Free car seats or boosters
A 24/7 nurse line
Access to the Healthwise® Knowledgebase online resource to check symptoms or learn about health conditions
A member walk-in center to talk in person about what you need
A health club membership discount at participating locations
