Hennepin Health Special Needs BasicCare (SNBC) is a health plan designed to meet the needs of Hennepin County residents with disabilities who are eligible for Minnesota Medical Assistance (MA). When you choose Hennepin Health SNBC you have access to healthcare services, specialty providers, as well as care management and transportation services within the Hennepin Health network.
SNBC is for adults.
You must:
Have a certified disability • Be a resident of Hennepin County • Be at least age 18 and under age 65 • Be eligible for Medical Assistance
