Background

Hennepin Health made updates to 2023 prior authorization requirements. A summary of these updates is below:

1. Hennepin Health removed the notification requirement for members in a hospital observation setting more than 72 hours.
2. Hennepin Health added a prior authorization requirement for hysterectomy when performed solely for the purpose of voluntary sterilization.
3. Hennepin Health clarified the requirement for “Injectable medications administered in an office setting” to include the hospital outpatient setting.
4. Hennepin Health added Sublocade to the injectable medications list.
