Housing Stabilization Services providers will need to follow Department of Human Services (DHS) requirement to submit newest version of Professional Statement of Need (DHS-7122) beginning July 17, 2023.

Housing Stabilization Services providers and qualified professionals must complete housing stabilization service assessments using the newest version of the DHS Professional Statement of Need (DHS-7122) (PDF) beginning July 17, 2023. DHS posted the newest version of the Professional Statement of Need (DHS-7122) in October of 2022.
For providers who have a Professional Statement of Need assessment completed on an outdated form before July 17, 2023, the assessment is still valid for up to 12 months and will be accepted as documentation when submitting Housing Stabilization Eligibility Requests (DHS-7948).
