The Family Home Visiting (FHV) program is a partnership between Hennepin County, providers, and local community agencies to offer free home-based services to eligible county residents. Public health nurses and other professionals visit families at their homes or community spaces to provide emotional, social, health-related, and parenting support and connect them with appropriate resources. The program aims to promote a healthy start for Minnesota children through positive birth outcomes and healthy child development, while also strengthening and restoring critical bonding relationships for families. The visits promote family bonding, provide prenatal and postpartum support, help families adjust to a new baby, prepare kids for school, and more. Referrals are accepted from families, health professionals, and organizations, including health plans, nurses, physicians, social service providers, and WIC programs. Self-referrals are also accepted. Referrals can be made via the family home visiting referral form.
Hennepin Health’s pregnancy webpage aims to help members learn about prenatal and postpartum care, conditions to be aware of, resources, and more.
