Rewards program

We think it’s especially important to get the preventative health care you need. Preventative, routine and follow-up care protects and improves your health.

Get rewarded for staying up-to-date with your health care. Take action for better health and a boost to your wallet. 

Getting rewarded is easy. 

1. Complete the visit or service(s). 
2. Give the voucher to the provider to sign. Having the voucher with you at your appointment is faster but you could mail it to them later. 
3. Mail the completed voucher back to us.
4. We will mail your gift card to your home.

Rewards you can earn

To earn gift cards:

• You must be a member at the time of the visit.
• You must be a member when the voucher is redeemed.

Vouchers are good within 1 year of service.

Adult

Annual dental visit
Members (ages 1+) can earn a $25 gift card for an annual dental visit by a network dentist.

Cervical cancer screening
Members (age 21-64) can earn a $50 gift card for completing a cervical cancer screening.

Colorectal cancer screening
Beginning January 1, 2023, members (age 45-75) can earn a $50 gift card for completing a colorectal cancer screening.

Diabetes diagnosis visits
Members (18 years or older) who have a diagnosis of diabetes can earn a $50 gift card for completing an eye exam and a $25 gift card for completing an A1C test.

Mammogram screening
Members (age 40-74) can earn a $50 gift card for completing a mammogram.

Pregnancy prenatal visit
Members can earn a $75 gift card for a prenatal visit that occurs in the first trimester of pregnancy or within the first 42 days of enrolling in Hennepin Health.

Pregnancy postpartum visit
Members can earn a $75 gift card if they have a postpartum care visit 7-84 days after the birth of their child.

Child and teen

Adolescent immunization
Beginning January 1, 2023, members can earn a $25 gift card for completing one meningococcal vaccine (MCV), one Tdap vaccine and two HPV vaccines before their 13th birthday.

Members (ages 1+) can earn a $25 gift card for an annual dental visit by a network dentist.

Child & Teen Checkup visits
Members can earn a gift card for your child's checkup visits:

• $75 gift card for six visits before 15 months at 0-1 month, 2 months, 4 months, 6 months, 9 months and 12 months
• $25 gift card for each visit for each visit at 15 months, 18 months, 24 months and 30 months, and each year ages 3-17.

Early childhood immunizations
Beginning January 1, 2023, members can earn a $25 gift card for completing the early childhood immunizations listed on the voucher before age 2.

Free car seats 
Members can get a free car seat or booster for babies and children (up to 8 years old) upon completion of a car seat class.

Young adult checkup visits
Members can earn a $25 gift card for each annual visit ages 18-21 years.
