Before providing services to Hennepin Health members, you must be contracted with us. Any claims submitted before an effective contract date will be processed as out of network or may be denied.
A request for a contract is not a guarantee that you will receive a contract. The contract assessment process takes up to 30 calendar days.
1. Complete the Provider Information form online at our no-cost provider portal, eApply.
2. Complete the following documents and submit them during you eApply registration. All providers must complete the following:
If you are approved to contract, you may be required to be credentialed or have a site visit. Our contracting and credentialing teams work in tandem to complete this process.
